  • Fill up the Application form downloaded from 'Application Forms' link under 'Information Desk' menu in this web site (except for Fine for Disciplinary Action, Due Amount and RTI)
  • Click on the 'Click here for Payment' button placed at the bottom of this page for remitting fee
  • Download the Payment Receipt
  • Send the filled application form, supporting documents and fee receipt to this Office
